We know these are challenging times for you and your loved ones. Like you, our utmost concern is for the health and well-being of each of our patients and to ensure our staff is healthy and able to continue to care for patients. With that being said, we continue to take all necessary cautionary measures and are strictly adhering to and implementing all CDC recommendations and guidelines. We continue to also work with local and state agencies as we fight Covid19. During this time we also recognize how important it is to keep the lines of communication open to give you peace of mind while we continue to meet the care needs of your loved ones at our facility. As such, the facility will be providing updates on new developments as they take place on this page to keep you updated.

December 14, 2022

Monte Vista Healthcare Center is currently working with LA County Department of Public Health to investigate a possible respiratory Outbreak or Influenza-like illness and to put control measures in place. We have tested all residents with symptoms for Covid-19, Influenza A&B and RSV. We do have 7 residents with RSV since 11/28/22 and at this point 0 residents, and 0 staff positive for COVID-19 as of today.
The Influenza virus is spread by droplets when an infected person coughs, sneezes or talks and the droplets land in the mouths or noses of nearby people.

Respiratory outbreaks during influenza season within skilled nursing facilities and other community care settings are commonly reported to the Department of Public Health. You are being notified in the interest of public awareness and safety.

We have already taken appropriate steps to prevent further transmission and eliminate the outbreak. Physicians and staff who provide care are aware of our status. Public Health is working closely with us to investigate the cause of the infections and administer the appropriate treatment or prophylaxis if appropriate. Staff education and strict hand washing for all staff have been reinforced. Vigilant infection control measures we already have in place can reduce the number of new infections.

Updated guidance as of 12/7/22 all visitors are expected to test within 24 hours before an indoor visit. If testing is not done prior to arrival at the facility, the facility will offer an antigen test that must be done prior to entry. Please sign in at the reception area or with a nurse.

Please allow this email to serve as a notification. Currently, there is one resident and no Staff that has tested positive for COVID-19 in the last two weeks. We are continuing to test 100% of staff and residents on a weekly basis. In the first round of testing, all staff and residents were negative except the one positive resident. We have tested all staff and residents for our second round of testing. If all are negative, we may be out of this outbreak as soon as Saturday 10/15/22.

The facility is continuing to implement all the necessary safety precautions that are consistent with the CDC guidelines as well as those of the recommendations of the Department of Public Health. October 8, 2022

Since the beginning of this outbreak on June 23, 2022, we have had 19 residents and 15 staff test positive. We are testing 100% of staff and residents on a weekly basis. If they have had a positive result in the last 90 days they will not be tested. Unfortunately, We had a resident test positive yesterday. This means we will not be cleared from our outbreak for at least another 14 days.

2021 COVID Updates

December 15, 2021

Monte Vista Healthcare Center is currently working with LA County Department of Public Health.

All residents and staff have been tested for COVID-19 and all results are Negative.

We have been cleared to allow visitation and our outbreak status is cleared as of December12, 2021.

Visitors will now be required to provide not only proof of vaccination but also a negative PCR test with 72 hours of visit or a rapid test the day of visit. LA COUNTY NEW ORDER will be in effective from December15, 2021 to January 31, 2022.

The visitation will be scheduled visits until further notice. The available HALF HOUR visitation slots are as follows:

Monday – 10am-12pm

Tuesday – 10am – 12pm

Wednesday – 1pm-3pm

Thursday – 5pm – 7pm

Friday – 1pm – 3pm

Saturday – 10am – 12pm

Sunday – 10am – 12pm

Visits will be done by appointment only and for 30 minute visits.

We will have four slots available per day for visits. If you would like to fill one of these slots please make an appointment with Rebecca (Marketing Director) Monday -Friday from 12pm – 2pm or e-mail her @ Rebecca.Murphy@Montevistahcc.com.

The available half hour visitation slots are as follows: Saturday – Tuesday 10am-12pm Wednesday and Friday from 1-3 Thursday evenings from 5pm-7pm

In response to increasing COVID-19 hospitalizations due to the highly contagious Delta variant, and to further protect vulnerable Californians and health care workers, the California Department of Public Health (CDPH) issued two new state public health orders Aug. 5. The first order requires workers in health care settings to be fully vaccinated. This order builds on Governor Newsom’s recent announcement requiring state workers and workers in health care and high-risk congregate settings to demonstrate proof of full vaccination. The new order applies to workers in hospitals, skilled nursing facilities, intermediate care facilities and most other health care settings. The second public health order directs hospitals, skilled nursing facilities, and intermediate care facilities to verify that visitors are fully vaccinated or have tested negative for COVID-19 in the prior 72 hours. Currently, there are no residents and Staff that have tested Positive for COVID-19 in the last two weeks. We are continuing to test 100% of staff two times a week and 10% of residents on a weekly basis.

January 20, 2021

Currently, there are no residents and Staff that have tested Positive for COVID-19 in the last two weeks. The last Positive test was December 28, 2020, for an employee. The last positive test for a resident was December 3, 2020. We are continuing to test 100% of staff two times a week and 10% of residents on a weekly basis.

Currently, there are 19 residents and 23 employees to date in the facility who are positive for COVID-19. We are testing 100% of staff two times a week and 100% of residents on a weekly basis.

Currently, there are no residents in the facility who are positive for COVID-19. There is however one case of COVID-19 positive employee this week. The employee has been instructed to Quarantine for 14 days. We are testing 100% of staff on a weekly basis. All staff and residents have been tested.

Currently, there are no residents in the facility who are positive for COVID-19. There is however one case of COVID-19 positive employee this week. June 12, 2020: There have been no new cases of COVID-19 positive residents or employees. We have 4 residents that are COVID positive, which are currently asymptomatic. They are in our isolated wing, with designated nursing staff. The facility will continue to implement all necessary safety precautions that are consistent with the CDC guidelines, as well as the recommendations of Public Health. We will continue to ensure the safety and well-being of our residents and employees. We will keep all lines of communication open with you and your family.


June 5, 2020: There have been no new cases of COVID-19 among residents and staff at the facility. We have admitted 3 new residents that are COVID positive, which are currently asymptomatic at this time. The residents are in the designated unit with designated staff. We, at Monte Vista Healthcare Center, will continue to implement all the necessary safety precautions that are consistent with the CDC guidelines. The safety and well-being of our residents and employees is our priority. Please be assured that during this time we will keep the lines of communication open to give you peace of mind.

May 29, 2020:

At this current time, there have been no changes from our last communication. There have been no new cases of COVID-19 positive residents or employees other than what was reported last week.

Although, we have admitted 4 new residents that are COVID-positive, which are asymptomatic. They are in our separated, isolated wing, with designated staff. May 16, 2020: At this current time, there has been no change from our last communication. There have been no new cases of COVID-19 positive residents or employees other than what was reported throughout the week. May 8, 2020: In the past week, there has been one resident who tested positive for Covid-19. No staff members tested positive last week. As of today, there are 26 COVID-positive patients in-house. They are all on strict isolation precautions as per guidance from public health. We will continue to monitor all residents for signs and symptoms of Covid-19. A total of 23 staff members have tested positive and will only return to work if given clearance from their physician. Know that we continue to monitor all staff members before and after each shift for signs and symptoms of Covid-19, including taking each person’s temperature.
